Community Relations

Our Community Relations programs provide a variety of services designed to support our patients’ overall health and wellness goals:

  • Care Coordination: Care coordination services are available to all patients at Koʻolauloa Health Center but we have found those with chronic health conditions, such as diabetes, hypertension, and asthma, find this service particularly helpful. Care coordination assists patients in the coordination of services to specialty care or ancillary services to minimize health care gaps.

  • Outreach & Eligibility: Outreach & Eligibility services provides clients direct assistance with applying for health insurance, financial assistance, the food stamp program, and health insurance coverage through healthcare.gov. Services are available at our Kahuku locations and our team can also be found at local events, fairs, and partnering with other area agencies.
