Community Relations

Ho`opi`i Ola Pono: KHC’s Ho`opi`i Ola Pono program provides a variety of services designed to support our patients’ overall health and wellness goals:

  • Tobacco Cessation: Services are provided by Certified Tobacco Treatment Specialists who work individually or in groups to support those trying to stop tobacco or vaping use. Free counseling, support and quit tools (such as nicotine gum, patches, prescribed medications, and more) are available
  • Diabetes Prevention Program: Lifestyle & Wellness Coaches provide group and individual support services for those who have been told by their primary care provider that they are pre-diabetic or at risk for Type 2 diabetes. The program uses a curriculum developed by the Centers for Disease Control & Prevention and includes fun learning experiences and activities. The program includes free classes and home support tools
  • Care Coordination: Care coordination services are available to all patients at Ko`olauloa Health Center but we have found those with chronic health conditions, such as diabetes, hypertension, and asthma, find this service particularly helpful. Care coordination assists patients in the coordination of services to specialty care or ancillary services to minimize health care gaps
  • Outreach & Eligibility: Outreach & Eligibility services provides clients direct assistance with applying for health insurance, financial assistance, the food stamp program, and health insurance coverage through healthcare.gov. Services are available at our Hau`ula and Kahuku locations and our team can also be found at local events, fairs, and partnering with other area agencies
