Job Title: Licensed Clinical Social Worker

Status / Time: Full-Time
Reports to: Chief Medical Officer
Position Type: Permanent
FLSA Classification: Exempt


Primary Function:
The provider’s primary responsibility will be to provide patient care in a holistic and culturally sensitive manner, following the latest clinical guidelines and evidence-based medicine, in a team-based, patient-centered approach.

Clinical Duties:

  • Conducts intakes, interviews, and screenings of patients.

  • Perform assessments of patients for diagnosis of mental health, substance abuse, trauma, or psychosocial needs.

  • Develops patient care plans that are coordinated and integrated with a comprehensive range of medical and psychosocial services to address the social, emotional, and practical needs of the patient.

  • Provides psychotherapy, counseling, or psycho-education utilizing individual, family, or group modalities using evidence-based interventions.

  • Consults and coordinates with primary care providers to enhance understanding of the patient needs and provides support for treatment approaches that incorporate behavioral health needs.

  • Assists other providers in the treatment and care of patients specifically in mental health or substance abuse treatment, especially in the context of the patient-centered medical home model.

  • Uses telehealth as needed.

  • Provides thorough and complete documentation of patient history, physical, assessment and plan as well as telephone calls and other miscellaneous patient interactions.

  • Accurately codes medical visit so as to maximize revenue and accurately reflect the work that is completed.

  • Appropriately refers patients out to various specialists for care beyond their scope of practice.

  • May supervise behavioral health students in a clinical setting.

  • Returns patient calls in a timely fashion, preferably by the end of the business day.

  • Utilizes nationally established guidelines and protocols for clinical decision-making and management.

  • Serves as a patient advocate in identifying health care needs and actions.

  • Maintains patient confidentiality and abides by all HIPAA rules and regulations.

  • Develops and implements behavioral health care education and health maintenance protocols in accordance with published standards, in collaboration with the other providers and members of the health care team as requested.

  • Actively participates in the quality assessment and improvement process, generating ideas, conducting assessments, peer reviews, developing improvement plans, and implementing and evaluating plans in collaboration with the Quality & Performance Administrator, other Providers, the Chief Medical Officer, and the Chief Operations Officer.

  • Completes progress notes and other necessary documents, i.e., patient encounter charge sheets, insurance forms, etc., in a timely fashion as per protocol, preferably within 24 hours and definitely within 72 hours.

  • Complies with and keeps abreast of current clinical policies and procedures. Keeps abreast of the latest advances in medicine. Keeps up with current standards of practice and recent literature.

  • Complies with regulations set by various grants.

  • Be available to patients after hours via an afterhours call service, 24 hours a day, 7 days a week. Assists with arranging coverage while on vacation or off-island.

Other Duties:

  • Participates in staff meetings, morning huddles, and provider meetings.

  • Participates in chart and peer review.

  • Participates in Ko‘olauloa Health Center (KHC) events.

  • Establish and maintain high standards of practice among clinic staff. Serves as a team player and role model to other employees in the organization.

  • Performs other duties to support the goals and objectives of KHC as deemed by the Chief Medical Officer or Chief Operations Officer.

Work Environment:

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.

The employee is occasionally required to stoop, kneel, crouch, or climb stairs. The employee must occasionally lift and/or move up to 50 pounds.

The employee must display a professional demeanor during periods of high stress (such as the ability to follow instructions; perform simple and repetitive tasks; adjust work pace to workload; perform complex and varied tasks; relate to people; influence people; make generalizations and decisions without direct supervision; and to accept and carry out responsibilities for decision making and planning.

Position Type/Expected Hours:

This is a full-time position and hours of work and days are Monday through Friday, 8:00 AM to 6:00 PM. Hours and days may vary depending on department and organizational needs.

Travel:

Travel is primarily local during the business day, although some out-of-the-area travel may be expected. Independent transportation is required to travel to various agency sites.

Required Education/Experience:

  • A master’s degree in social work from an accredited college.

  • Hawai‘i clinical social worker license.

Other Education/Experience

  • Basic Life Support certification.

  • Electronic Health Record (EHR) experience preferred; will be required to chart in EHR.

  • Ability to use email.

  • Familiarity with the communities to be served is preferred.

  • Good communication skills.

  • Must have good organizational skills.

  • Credentialed and privileged before practicing with the health center.

  • Ability to work with diverse cultures.

  • Three (3) references.

Competencies:

  • Current licenses and certifications on file.

  • Submission of continued education.

  • Chart review quarterly.

  • Annual evaluation.

  • Verification through a credentialing service.

  • Checking of National Practitioner Data Bank.

  • MS Office, Word, and Excel.

  • Excellent communication skills, both verbal and written.

  • Excellent organizational skills and the ability to define problems.
