CDC Prevent T2 Program*: Case Study 1

 
 

In March a new CDC Prevent T2 cohort was developed reinforcing the new implementation plan that lead us to better program outcomes. Through the new cohort we were able to reach 21 more individuals through weekly interventions. This Cohort has recently graduated and has successfully completed the 16-week program. 

At the end of the program 90% of participants reported engaging in healthy eating along with increased physical fitness activities. The average weight loss across all participants were about 16 pounds. It was also reported that participants were averaging about 183 minutes of physical activity each week at the end of the program. Over the course of time we were also able to identify improved measures in A1C and blood pressure for those who had a diagnosis of hypertension or diabetes. Many Participants were able to share their personal success through summaries that were submitted to the Programs Manager.

CDC Prevent T2 Program*: Case Study 2

 
 

“My whole life I struggled with my weight. I tried so many programs, read a lot of books, and watched a lot of videos of how to lose weight. I knew how to do it, I just didn’t have the will power to accomplish it. I was already considering doing the Gastric Bypass Surgery. My wife told me about the diabetes prevention class and her wanting me to come with her to check it out at an informational meeting. I reluctantly went and sat there thinking how I was going get out of it. I didn’t want to go because I was shame of my poor health. As I sat there I decided to go for it, I had nothing to lose. When I came to the first class I was introduced to a program that answered all my questions in an open and nonjudgmental environment. I met others who was in the same struggle as me. I also realized that these were my neighbors. The more I came the more I applied the program to my life. Before the start of class I was 322lbs.

As I applied the knowledge that was reintroduced to me, I began to make changes in my diet along with increasing my physical activity. Every week I saw changes, both physical and mental health. Logging in my three daily logs was a chore but as I did it faithfully which made held me accountable and my weight loss continued to progress. I now weigh 282lbs and I’m feeling so much better. I am happy to report I lost a total of 40 lbs. in the 16-week program! The program helped me make healthy choices that I will appreciate for the rest of my life. This is a great program and will be recommending it to all my unhealthy friends and family.”

One-on-One Nutrition Counseling

 
 

To sustain access to nutrition education and services within our community, our Dietitian offers services for individual nutrition counseling at Ko‘olauloa Health Center. From January through mid-June, 50 hours of individual nutrition counseling were completed within our clinic. These hours included time spent on initial nutrition assessments for new patients, along with follow up nutrition assessments. Initial nutrition assessments were one hour and follow up nutrition assessments ranged from half to hour visits.  

In addition to being active within the clinic, our Dietitian has played an integral role within our programs. She helped co-facilitate our 6-week Diabetes Self-Management program, along with facilitating four classes for our Lifestyle and Wellness program. One successful activity was held at Laie Shopping Center, where participants were given a $10 stipend to purchase a meal following newly learned nutrition guidelines. This activity allowed the group to expand on label reading skills and develop proficiency on choosing healthier meals when eating out.

 

We have enhanced our Programs by offering a healthy meal before each class night. Our Dietitian collaborated with support staff to develop a menu using program tools. Each meal was intended to allow participants to taste new foods, gather meal ideas for personal use, and create awareness of portion control. We aimed for a different low-cost meal each class- breakfast, snacks, lunch, or dinner meals. 

Although we are proud of these accomplishments, we acknowledge there is always room for improvement. The ongoing challenge for referrals to both individual counseling and Programs continues to be communication with our Medical Providers.  


*Prevent T2 is a CDC-recognized lifestyle change program designed to help people who are pre-diabetic cut their risk in half for developing type 2 diabetes. This program is also referred to as the National Diabetes Prevention Program.
