Did you know that thanks to a continuous emphasis on delivering patient-centered care that results in positive outcomes, Community Health Centers like Ko’olauloa Health Center have received bipartisan political support for more than 50 years. Although the Health Center Program originated at the federal level, widespread legislative support for Community Health Centers exists across state and local governments here in Hawaii as well. Building a stronger, more sustainable Community Health Center movement is dependent upon continuous engagement with stakeholders at all levels of policy. Patient Appreciation Day

Its Wednesday, 8/7 – Patient Appreciation Day

For the first time in history, Community Health Centers like Ko’olauloa Health Center served more than 30 million patients in 2021. A legacy of more than 60 years in the making, the Community Health Center movement began as a journey of building power among people within their unique communities. Today, Community Health Centers represent the largest network of primary care providers in America. Community Health Centers serve 1 in 11 people in the U.S. including:

1 in 9 children and adolescents
1 in 7 racial and ethnic minorities
1 in 6 Medicaid beneficiaries
1 in 5 uninsured persons
1 in 3 people in poverty

Additionally, federal policy requires that at least 51% of Community Health Center board members be patients of the health centers they represent. Consumer board members volunteer their time and expertise to be a voice for others who are served by the health center in terms of race, ethnicity, gender, and socioeconomic status.

Today we celebrate the patients and community board members who keep Community Health Centers accountable for meeting the needs of local communities. Mahalo!

Agricultural Worker Health

Did you know that the 2024 year marks the 62nd anniversary of the passage of the Migrant Health Act, the landmark agreement that sowed the seeds for what would later become the Community Health Center Program.

Community health centers like Ko’olauloa Health Center serve approximately 20% of the estimated 4.5 million Agricultural Workers in the United States. Health center staff members, community health workers, clinicians, executives, consumer board members, and Ag worker advocates have come together to develop more effective strategies to increase access to care for migratory and seasonal agricultural workers and their families through the Ag Worker Access Campaign.

The Ag Worker Access Campaign was launched in 2015 in partnership between the National Association of Community Health Centers (NACHC) and the National Center for Farmworker Health (NCFH). The Campaign’s goal is to increase the number of Agricultural workers and their families served in health centers to 2 million. Migrant Health Program grantees and community health centers, in general, are critical to ensuring access to quality primary and preventive care for patients who might otherwise go without care.

Health Care for People Experiencing Homelessness

Monday, August 7 - Health Care for People Experiencing Homelessness Day

Although all Community Health Centers provide care to vulnerable people in their communities – including some experiencing homelessness – more than 200 organizations receive targeted funding from the Health Care for the Homeless (HCH) program to meet the needs of those living without stable housing. Collectively, Community Health Centers, like Ko’olauloa Health Center, serve nearly 1.3 million individuals experiencing homelessness each year, a vast majority of whom seek care from HCH grantee organizations.

People who experience homelessness endure higher rates of chronic and acute disease, behavioral health conditions, and other needs that make them vulnerable to poor health, disability, and early death. HCH programs provide high-quality, comprehensive primary and behavioral health care, case management, and other supportive services to meet the needs of people who lack stable housing. Services focus on trauma-informed care practices that provide healing, choice, safety, and trust while empowering individuals as advocates in their own care.

The National Health Care for the Homeless Council works to improve the delivery of healthcare services to people experiencing homelessness by providing comprehensive technical assistance and advocacy support to HCH grantees. National Health Center Week is a time to honor and celebrate the work being done at HCH organizations and to advocate for policies that end homelessness by providing comprehensive housing as a basic human right.

Health Care for the Homeless

National Health Center Week - Health Care for the Homeless Day

Aloha ‘Ohana!

Did you know that collectively, health centers like Ko’olauloa Health Center serve 1.3 million individuals experiencing homelessness each year, with about 1 million of these individuals served by the 300 health centers that receive HCH funding?

People who experience homelessness incur a number of health issues on the streets or in shelters, and being without housing can exacerbate current health conditions. People without homes endure higher rates of chronic and acute disease, behavioral health conditions, and other needs that make them particularly vulnerable to poor health, disability, and early death.

Ko’olauloa Health Center provides high-quality, comprehensive primary and behavioral health care, case management, outreach, and other supportive services to meet the needs of people experiencing homelessness in our community. Due to the nature of homelessness, services are intentionally provided in trauma-informed ways that provide healing, choice, safety, and trust while honoring consumers as partners in care and experts in their own lives.

National Health Center Week is a time to honor and celebrate the work being done at health centers and to advocate for policies that end homelessness by providing comprehensive housing and health care as a human right.

National Health Care Center Week: Public Health in Housing Day

In today’s value-based care environment, organizations are accountable for improving health outcomes and lowering costs. To achieve these goals and succeed in such an environment, organizations need to better understand their patients to address both their clinical and non-clinical needs and impact the root causes of health, including patients’ health behaviors, health outcomes, and health costs.

The social determinants of health (SDH) are the conditions in which people live, work, play, and age. They can encompass socioeconomic conditions, environmental conditions, institutional power, and social networks. At Ko’olauloa Health Center we take time to understand our patients’ social determinants. This helps us transform care with integrated services to meet the needs of our patients, address their social determinants of health, and demonstrate the value we bring to patients, communities, and payers.
